On behalf of the San Diego Coalition for Compassionate Care, you are invited  to participate in a local convening of THE ROAD AHEAD For Serious Illness Care.   Light breakfast and networking will be from 8:30-9:00, with the program beginning at 9:00 a.m., with lunch also provided.

The Road Ahead presents and builds on the findings from the 2016 convening in Sacramento, “The Road Ahead for Serious Illness Care in California” is based on the 2015 Dying in California report,  and  the 2014 publication Dying in America: Improving Quality and Honoring Individual Preferences Near the End of Life, from the Institute of Medicine.  Judy Thomas, CEO of the Coalition for Compassionate Care of California, will kick off the day!

The purpose of The Road Ahead for Serious Illness Care is to bring together senior leaders and frontline workers who have ability to advance progress within their work setting or network.   We have included senior leaders from our region’s hospitals, physicians, skilled nursing facilities, Bio-Ethics, Payers, Hospice, educators, palliative care programs  and family members who participate in advisory roles to ensure a variety of perspectives and expand opportunities for collaboration.

There will be four central focus areas:

  • Delivery of Care
  • Clinician-Patient Communication and ACP
  • Professional Education and Development
  • Public Education and Engagement

Draft agenda:

  • The Road We’ve Been On—California
  • San Diego Region Accomplishments
  • Visioning: The Road Ahead
  • Assessing the Gaps and Opportunities
  • Report-out on Needs and Opportunities
  • Implementation of Next Steps

Please confirm your attendance as soon as possible.  There is limited seating of 50.
